Tips on How to Avoid Leakages #2

Too Infrequent Emptying:
Pouches should be emptied before they become half full. If they are allowed to overfill, the weight of
the affluent may break the seal and cause leakage.

Extremely High Temperatures:
Wafer melt-out may cause leakage in warm weather. More frequent changes will prevent this.

Pouch Wear and Tear:
Disposable wafers do wear out. If you are stretching your wear time, leakage may be due to the wafer
wearing out. Change your appliance more frequently.

Improperly Stored Appliance and Ageing Materials:
Store your ostomy supplies in a cool dry place, humidity may affect your pouch adhesive. Appliances don’t last forever. However, you can use expired appliances without damage to the peristomal skin.

Tips for Ostomates when Leaving the House

Try to practice the following tips:

1. Carry a small bag where you have ostomy supplies whenever you leave the house. In case of emergency, you have something to change with. Bring along wipes, alcohol and even an extra shirt.

2. When traveling by car, do not put ostomy supplies in the trunk where it is hot and the wafer can melt and be deformed.

3. Be cautious about food and water when you are in other countries.

4. In a place where weather is hot, be prepared to change more often.

Ostomy Wound Care tips

Caring for a Wound

Special care must always be taken when caring for a wound:
  1. Before dressing a wound, clean it thoroughly with sponges and cleansing solution. Do not allow scab to form in the wound which will cause infection. Use a gauze with saline solution to scrub of the wound. In case it bleeds, just put pressure with gauze until bleeding stops.
  2. Also, remove any body hair which also contributes to infection.
  3. Pat the wound dry with a different, clean sponge.
  4. When the wound is clean and dry, apply the dressings appropriate to the severity of the wound.
  5. When removing old dressings, always remove one layer at a time, not all at once.
Remember:
Calmoseptine and Desitine is for rashes and allergies while Silver Sulfadiazine is for wound treatment.

You can add anymore tips for ostomy wound care.

World Ostomy Day 2012

Counselling for Ostomy Patients / support group

I spoke with a patient having problems with the care of his stoma. Since he had no recourse what to do, he called for a Et nurse in a hospital along ortigas passing by Meralco Building. sorry but cannot mention the name of the hospital. The ET nurse charged 1000 pesos for a less than an hour consultation. He left immediately after prescribing stomahesive powder. He narrated that he has to visit 10 more patients for the day. That is a resounding 10k or about 220 dollars.

Kita mo nga naman ang mga iba nating kapwa nurses kung paano mang-abuso ng pasyente. Get in touch with us and we will charge you for free. It is ostomates helping us ostomates. After all, I believe that no doctor or et nurse will understand our situation more than ourselves.

Sad to say masyadong garapal ka naman sumingil pare kong ET nurse from M... hospital.

Using Stomahesive Powder for Excoriation

When you have weeping skin or technically called excoriation, here are some tips and inputs you could follow to dry the skin. You have to do this daily.

Put the powder on first then "blow" away the excess (not wipe). This will allow the powder to be absorbed by the skin where required.

It will not affect adhesion.

Then apply the barrier wipe.

When the flange covers the area, the skin heals for several days and when you remove the flange, the excoriation is gone.

Stool excoriation is the most common skin complication. It can lead to redness, weepy bleeding areas, pain, itching, stinging or burning sensations. Skin excoriation is often the result of stool coming into contact with the peristomal skin. Additional factors which may lead to excoriation include chemotherapy, radiation, psoriasis, adverse reaction or side effect to certain medications such as, steroids, antibiotics, chemotherapy, or immunosuppressives. A flush or retracted stoma can lead to stool leakage and excoriation. Poor hygiene, an inappropriate pouching system as well as allergies or sensitivity can also cause skin reactions.

The most common reason for stool irritation is an appliance or flange with an opening that is too large. Too much skin becomes exposed and stool comes in contact with it, causing irritation/erosion. The opening of an appliance/flange should be no more than 1/8 inch (3 to 4 mm) larger than the base of the stoma. This allows for adequate skin protection, but also ensures some "wiggle" room for peristalsis or normal movement of the stoma. The opening should not be snug against the stoma as this can cause a laceration or a cut to the stoma to occur. To ensure that the opening is correct, save the paper or plastic backing of the current appliance and place it over the stoma, ensuring that it is centered over the opening. Look in a mirror and check the gap between the base of the stoma and the edge of the opening. The stoma may need to be moved around (use a tissue and just gently move it from side to side, or up and down) to check all sides. If the opening is too large, then the pattern needs to be adjusted accordingly.

Once the cause of the stool irritation has been identified and corrected, then the actual treatment is quite simple. Using a pectin based powder (e.g. Stomahesive Powder™ or Premium Powder™), sprinkled lightly on the affected area, and then sealed with a skin sealant (e.g., No Sting™ or Skin Prep™) is usually sufficient. Using powder only may prevent the appliance from adhering to the skin, so a sealant is helpful. Usually, if all things are corrected, then healing or resolution should occur by the time the next appliance change is due.

Hop this helps!

Ostomy Nutrition Guide

Eating is one of life’s great pleasures. Having a stoma should not change your enjoyment of food. Most people with a stoma return to their normal diet soon after an operation. However food tolerances vary from person to person. The most important dietary concerns for the ostomist are preventing blockage and avoiding foods that produce unpleasant odors, gas or excess stool. Listed below are some general guidelines of the effects of foods after ostomy surgery. It doesn’t mean you cannot have the foods – you can still eat them but must be taken in moderation.

These are some general guidelines for eating which will help you care for your stoma more easily:
• Eat meals regularly. A stoma works best if you have three or more regular meals a day
• Chew your food well. If you have an ileostomy this helps prevent any food blockage
• Try new foods one at a time. If a new food seems to cause a problem eliminate it for a couple of weeks and then try again
• Drink a lot of fluid daily as you may lose more fluids than normal through your stoma

An extra note for ileostomists

It is very easy to lose large amounts of water, minerals and vitamins through an ileostomy when diarrhea occurs which can quickly leads to dehydration. When this occurs you need to replace these fluids so that you do not become ill.

Foods High in Potassium Foods High in Sodium
Milk, Beef, Fish, Pork, Peppers Tomatoes, Broccoli, Potatoes, Spinach, Green beans, Tomato juice,
Avocado, Apricots, Bananas, Cherries, Grapefruit juice
Strawberries, Pineapple, Dates Salt added to food, Canned and dried soups, Canned meat, fish, gravies, Pickles and relishes, Salted popcorn, Pretzels
Vegetables prepared in brine, Cold cuts of meat, Ham and bacon, Hot dogs, Peanut butter, Salad dressings, Sausages
Tomato juice and sauce

Esteem Synergy Convex Wafer and Bag

Convex Wafer and Bags

What is a Convex Wafer?

The special kind of wafer adjusts to the stoma inasmuch as it does not protrude but it is situated inside the abdomen. There are stomas that are flat and follows the normal contour of the stomach. This will require ordinary stomahesive wafers.

The use of convex wafers are especially helpful for the stoma that is deeply embedded. This will ensure that the wafer will stick and last long as it follows a convex shape.

See figure below.

I have excess convex wafers and I am giving it out for free. My stoma is not embedded but it lies flat. I will be most happy to give it to those who are in need. Please feel free to contact me so I can give you the wafer and bags.
Maraming salamat po.!
